How complex is care delivery in a transplant and cellular therapy program? 🤔 We often talk about this in abstract terms, but when you map it out, the reality is striking. Allo HCT/BMT is one of the most medically complex procedures that can be performed, but it also currently has *excellent* outcomes, so programs shouldn't shy away from implementation. It just takes expertise, great acute care facilities, and thoughtful long-term follow up (much of which can now be done via telemedicine). I created this figure using solid organ transplantation as a reference point to illustrate the layers of coordination, decision-making, and longitudinal management required. From donor selection to acute peri-procedural care to lifelong follow-up, the system is far more intricate than many appreciate. Curious how this resonates with others in the field. Donna was diagnosed with pancreatic cancer at age 66 when she and her husband were visiting one of their daughters in Australia. When they returned to the U.S., they immediately came to MSK for care, where she learned she was eligible for an investigational vaccine trial, to prevent pancreatic cancer from returning after surgery. MSK hepatopancreatobiliary surgeon Dr. @Jeffrey_Drebin, physician-scientist @TheVinodLab, and medical oncologist @EileenMOReilly walked Donna through each step. “They described how they would take part of my tumor to make a personalized vaccine, and it sounded amazing,” Donna says. “My husband and I had a moment of concern about delaying chemotherapy, but all the doctors made me feel so comfortable that we took the leap of faith and said, ‘Go for it.’” Over the next few months, Donna received an immunotherapy drug and eight doses of the vaccine, followed by chemotherapy, and then a final vaccine dose. Today, Donna is 72 and recently celebrated her 50th anniversary with her husband in Sicily and spends as much time as she can with her daughters and six grandchildren. Ozempic isn’t fixing your metabolism, it’s suppressing your appetite while the root cause stays untouched. By mimicking hunger-regulating hormones, these injections induce significant appetite suppression and delayed gastric emptying, effectively forcing a caloric deficit. However, this exogenous intervention does little to resolve the underlying biochemical signaling issues or the cellular inflammation that drives weight gain in the first place. When the focus remains solely on the scale rather than restoring metabolic flexibility, the body remains dependent on a chemical crutch while the root cause often rooted in hyperinsulinemia and poor mitochondrial function continues to simmer beneath the surface.
