My wife mailed in proof of her mother's death to VA election officials last fall. We found out last week she's still on the voter rolls and eligible to receive a mail in ballot. When we called, the county registrar apologized for the mistake and needed yet another copy of her death certificate to cancel her status, as if the first one would somehow change.
