everyone with acute, severe hypoxemia & bilateral lung infiltrates now has ARDS 🙄
we’re converting a trashcan diagnosis into a gigantic dumpster bin.
studies on this increasingly heterogenious group of patients will struggle to find any useful therapies.

New ARDS definition🫁 What's new👇 1⃣HFNO ≥30 L/m 2⃣SpO2/FiO2 ≤315 & SpO2 ≤97 3⃣Ultrasound diagnosis of bilateral opacities 4⃣Consideration for resource-limited settings But it's hard to know the consensus process from one-page abstract🤔 🔗atsjournals.org/doi/abs/10.116… #FOAMcc

Thanks to everyone for all the support over the last 6 years, in particular to @RachelCasas14, @sakshisummer, @Jed_Gonzalo15 and @SGandotraMD for convincing me to go through with this last year.

Doctors at a major US insurance company deny tens of thousands of claims a month without even looking at patients’ files.
“We literally click & submit,” a former Cigna doctor told @ProPublica & @Capitol_Forum. “It takes all of 10 seconds to do 50 at a time.”

@Ryan_WilsonMD @CZackMD @Mike_P_Pfeiffer @JohnBoehmer3 Provocative, but not practice changing. SCAI class D/E in 50% of context arm vs 20% in FlowTriever arm, almost certainly people chose to give tPA in patients who were more unstable on average, hard to capture in the data but already suggested by the above

To drain or not? how much? notice the LA colapse. Patient has 110mmHg of PSAP. #echofirst @CyntiaMachain @SONECOM_AC @NMerke @fazalabul @wikimagen @cardio_sonora @ecolabincich @Cardiotweets83

Agree with caution and not just draining all at once, but it can be safely done and would need to be if they have tamponade physiology.

@drmoneymatters @sethmhardy @ScottTruhlar This is my biggest concern. Talking to trainees over the years it's shocking how many get pushed into WL who aren't saving in their tax-adv accts. From an ed standpoint for beginners you get better learning/retention with a clear rather than ambiguous message.
English

@sethmhardy @chu020 @ScottTruhlar I think a good rule of thumb particularly for young physicians is to avoid it. And then if you have enough other insurance coverage and investment $$ allocated, then explore it. But it’s usually pitched to young residents by friends/family who are not looking out for them.

Per request, all of the talks I've done for the Personal Finance Series at #PennStHershey for our residents and fellows have been recorded and are now available on YouTube for easy access.

English

@sethmhardy Have to hold for 3+ decades to have an overall positive return, and you'd still be far behind compared to investing in a tax-efficient manner in index funds in a taxable brokerage account. Need a very specific thing that whole life can provide to justify the lower return.
English

@sethmhardy There are a lot of boxes you need to check that physicians won't come close to hitting, maxing out tax-advantaged space, having an estate tax issue ($12.06 mil for 2022, x2 if married, so probably not), needing asset protection for very specific reasons, etc.

Updated 2022 ERS/ESC PH guidelines, 2 is the new 3, among a few other changes.

@maahmed1218 @PennStHershey I'm just glad to have had the privilege of being your coach during your time here. Getting to work with you all is easily one of the best things about my job.
English

One of the best relationships I’ve had during residency has been with my coach @chu020. Also available and always allows us to have honest, open conversation. @PennStHershey has a great new pulm crit program director.
