@PulmCrit I continue to be confused why we can’t understand acid base better and break all this dogma.

I continue to be confused about why we can't #BeTheKidney more often - and give bicarbonate to intubated asthmatics to accelerate renal compensation.
the kidney will eventually do this for us, but it takes time (1/2)...

@nohandsup This repetition of misinformation is why blood gases are so badly done.

@WhistlingDixie4 @junesarpong @LizzieK101 is a role model to many (including myself) and I still try to channel some of her leadership style if I’m in a pickle. It’s been a while and roles have changed, but def worth a mention!
English

Reading a book by @junesarpong that discusses point that ‘men don’t see women as role models’
I have lots of male role models - never really considered whether men in medicine have female professional role models
Thoughts? Honest ones…not just what you think you should say…

@srrezaie Been using it for a while. Especially if a “hard diuresis phase” is needed for the very overloaded subtype. Keeps your chloride and acid-base in better balance.

@PulmCrit If you use only dobutamine when your pt has a degree of myocardial suppresion from sepsis and neglect the vasoplegia…

👇 agree… I *know* this isn’t backed up by high-level evidence, but I’ve found dobutamine to be disappointing in septic shock
epinephrine seems more useful in sepsis, especially for brady/⬇️EF/normolactic patients who aren’t making their own epi 😛

ANDROMEDA-SHOCK@AndromedaShock
Very nice article from Armand Mekontso-Dessap and three ICUs in the Paris area! More data challenging dobutamine use in septic shock. In my humble opinion it should be used only in limited situations, after en echo to rule out phenotypes where it may be harmful, and as a test!
