@mwassermannl Dear Professor Wasserman, I hope you are doing well. I’m following up briefly on my recent email invitation, in case it may have escaped notice. With kind regards, Jorge

Autonomic neural blockade can reduce postoperative visceral pain, nausea, and vomiting following minimally invasive surgery. It may also provide an important adjunct to enhanced recovery protocols. ja.ma/4f2W0sG

ANB targets autonomic pathways during minimally invasive procedures to effectively manage related visceral pain, nausea, and vomiting, promoting intraoperative hemodynamic stability, reducing anesthesia needs, and enhancing recovery. #SurgicalInnovation #AutonomicBlockade JAMA

However, it is a real and unwanted occurrence in a busy practice. Preventive measures are worthwhile.

@DrSumeetShah @ericpaulimd @OMaxFisher I have not studied diabetic patients particularly. The autonomous system is complex. Visceral pain and associated symptoms such as nausea, vomiting, perspiration and anxiety seem to be carried by afferent pathways shared by sympathetic and parasympathetic nerves.

@DrSumeetShah @ericpaulimd @OMaxFisher VP substantially impacts patients’ quality of life, recovery time, nursing time allocation, and resultant risk of opioid abuse. Nausea, food intolerance, and pain are responsible for most of the readmission after LSG and other bariatric procedures[6,7].

@daes_jorge @ericpaulimd @OMaxFisher Most of LSG patients are up & around the same day. Many are doing routine activities within 48 hrs. I don't find pain scores to be very different from LC or even Lap Extraperitoneal VHR. So, my question is why did you find the need to do this additional pain control procedure ?

@DrSumeetShah @ericpaulimd @OMaxFisher Autonomic impulses are, in part, responsible for the visceral pain and associated symptoms, such as nausea and vomiting, observed in a substantial number of patients in the immediate postoperative period after LSG and other minimally invasive procedures [1-5].

@ericpaulimd @OMaxFisher We are currently finishing a protocol on an RCT comparing ANB performed at the outset with at the end of the LSG. Performing the procedure at the beginning has been shown in an observational study to reduce the amount of remifentanil and halogenated anesthesia significantly.

@sbjonesmd @SEAnesHQ Respectfully, I would like your opinion on this investigation. Now we have started a protocol using this ANB at the outset of the procedure noting a remarkable reduction in remifentanil and sevoflurane consumption and hemodynamic stability.
