Vascular liver diseases are challenging. Subtle histology, evolving PSVD terminology, & biopsies that don’t align with the clinical picture can complicate diagnosis. At #USCAP2026, Dr. Fiel and I will discuss real cases using a simplified framework for confident decision-making.

3/ We found that these are a poorly-differentiated/solid variants of lung adenocarcinoma with massive accumulation of mitochondria, which makes them look granular, but also commonly vacuolated and clear. This does resemble some known eosinophilic tumors, like HCC, RCC, etc (image is EM showing accumulation of mitochondria)

1/ Just out in @ModernPathology New study from our group: A distinct variant of aggressive lung carcinoma with peculiar pathologic features. We called these tumors “SAGA” (solid and granular adenocarcinoma) .. sorting them out was in fact a long and winding saga that led to some unexpected insights. 👇 modernpathology.org/article/S0893-… pubmed.ncbi.nlm.nih.gov/41580239/

2/ For pathologists: Here is what they look like. You have seen them, and probably wondered what these are. For some, you may have wondered if they are hepatoid (they partially are but not quite), and some have posed a challenge with determining the site of origin – these are usually widely metastatic and consistently negative for lung markers (TTF1, Napsin).
