@Lady_Pheebs Wir können in der Medizin ziemlich gut feststellen, woran jemand gestorben ist. Altersschwäche ist keine Diagnose, die auf einen Totenschein eingetragen werden kann.

Vor zehn Jahren ist ein 85-jähriger Mann mit einer verschleppten Bronchitis, der im Sonnenschein spazieren gegangen ist und tot umgefallen ist, an Altersschwäche gestorben. Vor fünf Jahren ist derselbe Mann an Corona gestorben. Heute stirbt derselbe Mann als Hitzetoter.

You can connect 2 distal perfusion cannulae to a single #ECMO port using a 4-way stopcock or, like in this case, a 3-way stopcock after breaking the seal.

@Boris25309876 @GeoWieland Also ich kann dir aus meinem Umfeld nur sagen, dass die älteren/hochbetagten hier an dem Hitze WE in deutlicher Zahl gestorben sind. Ich hatte selber Dienst und das waren Kriegs ähnliche Zustände…

@GeoWieland @doc_ecmo Die kleinen zusätzlichen Zacken bei Volumenmangel gehen vermutlich auch auf Hitze zurück: x.com/Boris25309876/… Aber wenn am sich DARUM jetzt ernsthaft Gedanken macht, dann müsste man Senioren zum Selbstschutz letztendlich empfehlen permanent einen Helm 😄 zu tragen.

All patients with failing Fontan circulation must be considered for VAD or heart transplantation. Complexity must not be a reason for an exclusion!

- Hitzetote im engsten Sinne (ICD10-Todesursache sinngemäß Hitze) haben wir so um die 20-25/Jahr --> nicht aussagekräftig. - Viel Komorbidität. - Übersterblichkeit beinhaltet große Fehlerquellen. RKI berücksichtigt immerhin Temperaturen. Das ist weit besser als alles andere.

#I_tried_my_best Another PE , different outcome .. 27/M , smoker #Massive_Pulmonary_embolism, atleast 7 days old PE as per history and worsening since .. viral symptom onset 2 weeks ago .. now Shock & Near Systemic PA pressures .. Tried with PENUMBRA CAT 8 , then upgraded to 12F sheath .. Then upgraded pressures from -29 to -200 via suction pump .. Cud not remove RPA CLOT no matter what i tried.. @IM_Crit_ A decompensated (Fluid overload i.e.) or instable Patient with AF in RVR, almost never has a good LVEF. Else they wouldn't bei instable/decompensated. I think this should be taken inzo considerations before giving AV nodal blockers. #echofirst

Atrial fibrillation secret: When a patient presents to the ED, ICU, or wards with new-onset atrial fibrillation -and we do not know their systolic function- it is worth taking a quick look at the heart before reflexively reaching for diltiazem or metoprolol. We do not always need a formal echo before acting. But if bedside US is available, even a brief POCUS assessment can change the entire plan. I know this is not always feasible in real time. But every year, I see a few patients who crash -or even arrest- after AV nodal blockers are given, only for POCUS afterward to reveal an ejection fraction of 10-20%. The point is simple: not every patient with rapid atrial fibrillation has a ventricle that can tolerate negative inotropy. Let’s try to make the “diltiazem death challenge” a thing of the past.
