- 2022: Ozempic shortage starts. Clinical need puts glp-1s on the pharma shortage list. Compounding pharmacies: we can sell these legally for now - 2022-2024: 70+ telehealth companies flood in. Price competition drives compounded semaglutide from ~$400/month to ~$115 - late 2024: FDA pulls tirzepatide off shortage list - feb 2025: Pulls semaglutide off the list too - march 2026: 30+ warning letters. FDA instructs companies to stop implying their compound is equivalent to Wegovy - also march 2026: Novo Nordisk sued Hims & Hers for mass illegal compounding after they launched a $49 off-brand semaglutide pill - six weeks later: Big pivot - Hims & Hers announced a partnership with Novo to sell branded Wegovy and stopped advertising compounded alternatives - april 30: FDA proposes permanently removing all three GLP-1s from the 503B bulks list entirely - june 29: Public comment deadline -> if this holds, will be one of the largest resets in the history of healthcare (and has huge implications on Reta + upcoming pharma innovation) In ~48 months, the GLP-1 compounding market proved that millions of americans will cash pay for drugs that work. No prior auth, no PBM, no co-pay, and the implications will shape the future of pharma. Branded manufacturers are now building direct access programs because the compounders showed them the demand was there. Whoever launches the next major metabolic drug gets to do it into a patient base that's already educated, already motivated, and already used to paying out of pocket.
