1/8 🧵As an MD & software developer, I've been reflecting on @ONC_HealthIT's #HTI1 rule, which proposes recording patient preferences on data sharing without required providers to honor them. This can disappoint -- and it misses more pragmatic opportunities for patient control.

I have a growing conviction that the "standardization" of health data in this coming era will behave more like ELT -- a late binding transformation driven by very capable automated agents acting in the context of specific questions.

@JoshCMandel Love this @JoshCMandel! And won’t it be a great day when all the data from this and other #ClinicalTherapies is standardized and available as part of #HealthData #interoperability so that individuals and providers can gain a truly holistic picture of our health and illnesses?

Oh, GPT-3..5.... "the opposite" is not close enough. > This code... represents... the opposite of what the patient is experiencing. Therefore, it is not a perfect match for the user's query, but it is the closest match available in SNOMED-CT.

No one in the Midwest is talking about SVB, home prices, or yield curves. We are working, living, and consuming like normal.

@MPalantoni @healthapiguy Interestingly, Twitter chose to hide your reply even though it's the most relevant thing to show. I guess you really do need to buy visibility these days 🤷‍♂️ Thx for setting the record straight!

@healthapiguy No current plans to be QHIN on road map. Lots of work on interop experience and platform to platform connectivity. Plan is to connect to QHIN(s) to maximize access for clients, care teams + patients, just like national networks today where we do >1B exchanges/yr.

unsurprising, but hearing that athenahealth will be angling to be in the next crop of TEFCA QHINs. well-positioned as a Carequality Implementer supporting high volume with a centralized RLS today

@healthapiguy The 3rd point just isn't true, at least outside of a handful of cases. Payers are not supported by the existing networks b/c none of them push P&O.

some other interesting CMS Prior Auth comments @1up_health - an outright denunciation of TEFCA. unsurprising given their products and also the comment author. somehow manages to add in an equity angle (TEFCA has brokers, FHIR won't?) regulations.gov/comment/CMS-20…

overall, relatively low number of comments by affected parties (payers, state medicaid agencies, etc), so @CVSHealth PA comment also worth a closer review they support not including drugs and not giving out-of-network providers the provider access API, obviously

@sjresnick I almost lost escrow on a house because a local decided to deduct all losses of our venture backed c corp from my personal W2 to determine loan eligibility. This after promising me they understood business owners. Scared me away for business banking for sure.

WI’s tech ecosystem would be strengthened if more local banks stepped up. Plenty of WI banks make it difficult for founders to establish any line of credit. SBV meanwhile is offering financial management tools, virtual cards, and hosting networking events.

@g_redsten unearthed a major point that most WI press missed: why did WI founders choose SVB over locals? SVB understood the unique needs of growing startups. Many regional banks reject founders and their associated risks. Many don’t offer products catered to tech entrepreneurs.

3 - In addition to accepting deposits, the are actively engaged in venture lending. Said that today in fact they approved new loan arrangements. And will honor all existing loans - whether drawn or not.

In case helpful for a broader audience, a number of VCs were on a call tonight with the new SVB CEO. Here are a few data points they shared with the group:

@johngrund @JustinWolfers There is going to be a ton of retrospective analysis to perform and there is certainly some dereliction of duty to evaluate. Not trying to imply otherwise. This is too nuanced for quick Twitter takes.

@johngrund @JustinWolfers The "VC cash" was LP cash. The LPs are groups like CalPERS and Texas Teachers who hold retirement accounts for millions of normal people. That's not even the real issue. The companies holding deposits at SVB could be anyone's employer. Every business needs simple, secure banking

@JustinWolfers You know how this works. There have been too many hot takes the last few days from too many sides. Please don't fan the embers. Yes, we should prevent fires in advance. But once it's underway it's irresponsible not to contain the spread. (3/3)

@JustinWolfers They're you. Where do you think most of those deposits originated? Yes, HNW, family office and VCs benefit. But, so do the millions of teachers, first responders, union members and other pensioners who had funds wrapped up in these companies without even realizing it. (2/3)

@NeilThanedar 😞 some people are cruel. I hope they aren't a teacher, police officer, fireman, state employee or union worker. Lots of hard working Americans don't realize they have indirect exposure to SVB through their pensions and retirement accounts. This isn't a tech vs everyone issue.

When did tech become the enemy? I’ve got angry people in my DMs wanting us all to fail. Meanwhile I’m talking to founders who have struggled for 10+ years to get their startups to where they are now. And now they could lose everything. For them to go down like this is wrong.
