Some of my doctor colleagues are upset about this video, and I can understand why. To put it in perspective, you would not expect a plumber to supervise an electrician. In the same way, it feels like a stretch for a nurse, regardless of their title, to claim they are supervising a medical registrar in the NHS. Sometimes in allied health, we can become a little overzealous and overstep, and I think we need to call ourselves to order so we do not come across as ridiculous. To be fair, this is not necessarily the nurse’s fault; she was placed in that position by people who probably should not have done so. In principle, nurses should supervise nurses, doctors should supervise doctors, and physios should supervise physios. For example, I do not believe any doctor has the specialist skillset to supervise me as a physiotherapist, just as I do not believe any physiotherapist is equipped to supervise a medical registrar. What can and should happen is shadowing to learn specific skills or perspectives. During my joint injection therapy course, I spent three months under the supervision of a GP with an MSK interest, and I learned a lot. Likewise ( This is not a core physio role but an extension of the scope of practice to it makes sense), I have had GP registrars shadow me for the day. That is valuable, but it is not the same thing as formal supervision. The wider point raised about short staffing, however, is very real. That is not unique to one profession; it is an NHS-wide challenge that affects us all

The NHS is genuinely an awful employer. It’s been a little over a year since I left and joined a GP practice and it’s been wonderful. I look back at my time as a resident doctor, and particularly my time as co-chair of the BMA, and remember continuously reflecting on how bad things were. Your concerns are legitimate. The old guard doctors are often out of touch. The media don’t have a clue. The public can not comprehend. The NHS says a lot of utter nonsense and take no action - they either don’t understand or don’t care. THINGS ARE AWFUL FOR RESIDENT DOCTORS. The lack of agency. The lack of autonomy. The disrespect. The insane and illogical hoops you’re asked to jump through. Everything you hate is legitimate. Strike. You’re justified in doing so.
