Not all "fasted" patients are truly empty; As anaesthetists, we're constantly making critical judgement calls, particularly when patients present with unclear fasting status or are taking medications like GLP-1 agonists. Fortunately, gastric ultrasound can offer helpful insights to support our decision-making and guide safer anaesthetic planning. Key Messages ✅ Delayed gastric emptying is a real phenomenon - GLP-1 agonists can mask this risk. Gastric ultrasound helps unmask it, especially when history alone isn't sufficient. ✅ Rapid Sequence Induction may be the safer approach when ultrasound reveals solid or high-volume gastric content, even if the patient seems clinically "safe." ✅ Let ultrasound findings guide your choice between GA or RA, SGA or ETT.
