A hospitalized patient was diagnosed with several new conditions. He was being discharged with many meds, but was overwhelmed since he cannot read… Our wonderful resident spent time teaching him & color coded everything using pictures.. ☀️Take in AM 🌙Take in PM #Care #MedEd
