🚨 New Research Published 🚨 Proud to share our latest work: REMOTE Study – Feasibility and Effectiveness of Telemedicine-Based Echocardiography Mentoring in Intensive Care, now published in the Journal of Cardiothoracic and Vascular Anesthesia @JCVAonline In the face of a national bottleneck, with 1,450 clinicians registered for FUSIC Heart accreditation and only 190 approved supervisors, our study explored whether real-time remote mentoring could be part of the solution. 📊 Key findings from our mixed-methods study: ⚙️ 100% of planned mentoring sessions completed successfully ⚙️ Minimal technical disruption, with only 3 brief (<2 min) connectivity drops across all sessions ⚙️ High image quality (mean 1.90/2.00) and excellent diagnostic accuracy, with perfect scores for 5 of 6 parameters ⚙️ Outstanding teaching ratings, with a mean of 5.93/6.00 on the Clinical Teaching Assessment Form ⚙️ Strong participant endorsement, described as “game-changing” in sites with no mentors and “more 1:1 and personalised” than traditional bedside teaching 💡 Why this matters: Remote mentoring has the potential to bridge geographical training gaps, expand access to expert supervision, and maintain high educational standards, even in complex ICU environments.This is just the start, and we now need to explore scaling this nationally, integrating hybrid models, and leveraging AI to further enhance training and diagnostic support. 🔗 jcvaonline.com/article/S1053-… #CriticalCare #Echocardiography #POCUS #Telemedicine #MedicalEducation #Ultrasound #Research #FUSIC

🔍 Breaking paradigms in critical care ultrasound: New insights challenge traditional E/e' measurements in cardiogenic shock 📌 Fascinating research from Barts Heart Centre challenges our conventional approach to estimating left heart filling pressures in cardiogenic shock. In a analysis of 40 patients with acute MI and shock, left atrial volume emerged as a superior predictor of wedge pressure compared to the traditional E/e' ratio. 🤔 Why does this matter for bedside critical care? Whilst we've traditionally relied on E/e' measurements, this study suggests we might need to shift our focus. Left atrial volume showed a moderate but significant correlation (r=0.433, p=0.007) with wedge pressure, while E/e' showed no meaningful correlation at all. 📌 The practical takeaway: A left atrial volume >54mL demonstrated 67% sensitivity and 65% specificity for predicting elevated filling pressures (PCWP ≥15mmHg). This could be game-changing for centres without immediate access to pulmonary artery catheters. The findings make physiological sense - acute left atrial dilation appears to be an early response to elevated left ventricular end-diastolic pressure during acute MI. This offers us a potentially more reliable marker for left ventricular loading in our sickest patients.
