In 1981, Dr. Michael Shannon worked tirelessly through the night to successfully rescue a premature baby boy. 30 years later, in 2011, Dr. Shannon was trapped in his burning SUV following a collision with a semi-truck. Miraculously, he was rescued from the blazing vehicle by Chris Trokey, a paramedic with the Orange County Fire Authority. Remarkably, it was revealed that Chris Trokey was the same premature baby Dr. Shannon had delivered three decades earlier.

Intubation attempt #1 failed, and now you have to bag them back up. This technique has been referred to as the "silo" or "barrel" technique. I prefer to call it the "bowling ball" 🎳 technique. Placement of 2 NPAs and an OPA significantly increases the cross-sectional diameter of the airway that is stented open. This will prevent airway tissue collapse as you apply external positive pressure. The safety pins shown are to keep the NPA from getting dislodged in the airway if the flared end is smaller. This is not usually necessary with the newer designs.
