Tested a few AI models on this case. All models agree the automated “atrial flutter” read is likely incorrect, and this is a regular tachyarrhythmia in a stable 9-year-old requiring urgent cardiology-aware management. The main split is **SVT/AVRT (including possible WPW)** versus **wide-complex VT**: Claude Opus favors SVT, Claude Sonnet favors SVT with aberrancy/WPW, while Gemini and GPT-5 lean toward VT, especially idiopathic fascicular VT. Great case yesterday. Pediatric patient with 9/10 chest pain/pressure and palpitations. BP wnl. Sats normal. No syncope. Otherwise healthy. No PMH or FHx relevant. Will post outcome in a bit. I want to hear your thoughts.

With the long QT and WCT we avoided adenosine and Amio. Gave Magnesium over 20 minutes and the QRS narrowed and her symptoms resolved, but still tachy >200. In the ED they hemmed and hawed for 20 min and finally decided to give adenosine. Final dx SVT with aberrancy.

@PulmCrit @akatzzzzz The PITSTOP RCT is pending by @DrCheskes and his group from Toronto. Should solve this debate. Septic shock - anywhere - needs fluids, pressors, and antibiotics, irrespective of location. Nothing magical happens in the hospital.

@akatzzzzz They're not supported by rigorous RCTs. They impair the ability to obtain cultures later on. Broad use of ABX will cause antibiotic resistance. May cause allergic reactions in the ambulance. Will distract from other priorities.

Surviving Sepsis 2026 is here & it's even more loony tunes than I was expecting. They're promoting pre-hospital ABX & preemptive broad-spectrum IV antibiotics for intubated patients. This insane fever dream is an antimicrobial stewardship nightmare. Embarrassment for SCCM.

@HandtevyMD @joshkimbre @FOAMfrat There is still a burden of proof on you to suggest that WB has any benefit over component therapy in your context of metro EMS with much faster response times. But I ain’t gonna argue that some interventions will work for you that won’t in protracted UK HEMS land.

SWiFT (n=616) randomized air ambulance patients in the UK requiring blood transfusion to whole blood vs. component therapy. Outcome: Death or massive (>9 units in 24h) transfusions. The primary outcome was not different between groups. Great work! nejm.org/doi/full/10.10…

@joshkimbre @HandtevyMD @FOAMfrat What I find irritating more than anything is when a US civilian ambulance service claims that a UK civilian prehospital study doesn’t apply to them deep down because they align idealistically with their military colleagues. It’s bad science and wrong. (See TXA, REBOA, etc)

Alan and his family invited me for a doubles match, on the same court where two bystanders, my crew from Palm Beach County Fire Rescue Station 49, and EMS Captain Darwin Zelaya, fought to bring him back. They delivered 2 AED shocks, 1 standard shock, six rounds of Dual Sequential External Defibrillation, as well as amiodarone and esmolol, achieving ROSC before arrival at Delray Medical Center. Alan also recorded a quick PSA for clinicians who still question whether #DSED works. Walking off that court, I couldn’t help but feel grateful. Grateful for the bystanders, the crew, the science, and for the chance to see a life fully reclaimed. @DrSamanthaF @AHAScience With all due respect to Charles, this one’s not even close. The AHA and ERC missed big here. @DrCheskes NEJM RCT showed a doubling of survival…..of people who would have otherwise died. This recommendation needs to change.
