@MorphologyAmigo @Dr_MSU @nihardesai7 @TalhaBadarMD @PriyaSriRajah @blood_academy @WilsonAndrewDr @BijoyTelivala Ivosidinib is an IDH1 inhibitor, so not applicable here. Enasidinib (IDH2 inhibitor) not a UK option at the moment…a case of treat with what you have…
English

@Dr_MSU @nihardesai7 @TalhaBadarMD @PriyaSriRajah @blood_academy @WilsonAndrewDr @BijoyTelivala @h4yder I think what @nihardesai7 and I are trying to say is
you can do anything you like
The idea of these regimes is quality of life and disease control
Fancy Heroics which will break the patient in half with side effects probably wont do him any favours
Monosomy 7 is bad luck

Chromatin patterns 🧵
[1] How many times have you heard phrases like:
"Fine chromatin"
"Dotted like with a pencil"
"Smooth chromatin"
"Clumped chromatin"
"like a football (some of y'all may call it a soccer ball)"...?
What do they mean and how do you make sense of it all? How does the chromatin relate to the morphology? Let's take a deep dive. 👇

@MorphologyAmigo @veraghali @NicoGagelmann Heard and read about plateletpharesis but have never had to do it, cytoreduction being the mainstay. Having said that I do know of a case where patient had a count of >3000 and resistant to max dose hydrea, pegasys and ruxolotinib…
English

@veraghali @NicoGagelmann With respect
The only Apheresis indication for ET is
If a pregnant woman has Very high platelets and is pregnant and somehow the newborn is under threat.

Very pleased to able to share our latest research on platelet activation in #VITT authored by @richardbuka and @sam_310 from @bhamplateletgrp and @ICVS_UoB published in @BloodJournal yesterday. We show that PF4 itself activates platelets via the TPO receptor c-MPL.

@ManniMD1 I thought that Sjogren’s is associated with MALT lymphoma which often has an IgM paraprotein. Although by definition this isn’t MGUS…

In case the link above didn’t work.
English

I don’t think MGUS is actually associated with Sjogrens.
Or many other auto-immune diseases for that matter.
sciencedirect.com/science/articl…
So, I will respectfully disagree with you there.
Sarah Schafer MD MPH@SarahSchaferMD
@ManniMD1 @VincentRK Always look for Sjogrens which can be associated with MGUS and causes neuropathy in many patients. Neuropathy patients are often SSA negative so you need to do more than a blood test.

@richardbuka Maybe best place to intervene is at pharmacy/dispensary level. It’s quite a good way of improving prescribing patterns…@nickduncan31?

This paper came out 9 years ago showing less is more when it comes to oral iron - once a day or alternate days is optimal. Yet, still patient after patient is on BD or TDS 🤢. WHAT DO WE HAVE TO DO TO STOP CLINICIANS DOING THIS? Just causes GI probs.

@MorphologyAmigo Have never come across this scenario! Broadly speaking allergies need close scrutiny-thorough history, speak to pharmacy and immunology. ATRA is so crucial though that I’d be likely to press on with it (maybe the dexamethasone cover for differentiation syndrome will ameliorate!)
English

I think Severe Soya Allergy and ATRA dont go well in the same cocktail
never been confronted by it myself but have read about it in books
Perhaps some myeloid friends might be able to cite their experience if ever faced with a situation like this ?
