Painfully ironic. Let’s look at the facts behind the noise. Two of the five “IMGs” everyone is upset about: Elena Ahrens — 43 peer reviewed publications, 400+ citations, Research Fellow in the department of Anesthesia at Harvard. Dr Adil Manji — 25 publications, over 1,000 citations, h-index 8, postdoctoral fellowship at Harvard Medical School. Both of them trained and worked at Harvard before residency. Ask any Program Director, they will tell you the same thing, when someone has already worked within your system, proven their competence, and demonstrated strong work ethic firsthand, they become a far more reliable and desirable candidate. This isn’t favoritism, it’s informed selection based on direct experience. But that nuance is lost on those who don’t even understand the fundamentals of graduate medical education. These are not “average” applicants. These are elite, globally competitive physicians who would be top choices in residency programs anywhere in the world. Residency selection is not about passports. It is about merit, performance, and potential. The real question isn’t who is “US-born” vs “IMG.” The real question is: Do patients deserve the most qualified physicians available? Because when the bar is this high, the answer is obvious. And for the record, anesthesiology is not some closed door specialty, many IMGs match into it every year.

I think you can accrue negative karma by not actively trying to let people know the potential side effects of a drug or anything that can cause them harm. Take cigarettes for example, there's a reason they let you know what can happen before you smoke them. The same goes for people who hand out protocols without addressing the potential downsides or tracking the side effects of what you're handing out. Abud might be coming across as an orator of peptides to some people, but he's just trying to extend his duty of care beyond the lens of his own patients, which is admirable. If more people had the attitude of safety over profit then the world would be a better place.

Some days I wake up and I’m like… do physicians actually enjoy getting their asses handed to them by a handful of mid-level bureaucrats at the AHA and FAH? I mean, have you read these bios? Seriously. You go through them and you’re waiting for the big moment—“Ohhh this is where the genius comes in”—and it never shows up. It’s just… some guy. Another guy. And somehow these guys have spent the last 20 years absolutely steamrolling an entire profession. Physician ownership? Gone. Reimbursement? Down for two decades. And the beatings? Oh, those are still very much on schedule. Then you talk to physicians about it. And what do you get? A thread. A post. A strongly worded LinkedIn essay. “Oh, this is outrageous.” Yeah? Then do something. You say, “Hey, why don’t we fight back? We hire like 10 legit lobbyists, get organized, push policy...” And immediately it’s: “Ahhh, I don’t know, reimbursement’s down…” “My kids are in college…” “Timing’s not great…” The timing is not great? When is it gonna be great? When they take the last dollar out of your pocket and invoice you for the privilege? It’s unbelievable. You’ve got one side playing full-contact politics, and the other side’s writing Yelp reviews about it. At some point, you gotta stop complaining and realize you’re not losing because they’re brilliant. You’re losing because they’re actually playing the game… and you’re sitting in the stands explaining why you can’t. In the meantime, another pharma company donated $50 billion over the next 10 years, and pro fees are down again, while facility fees are....UP UP and UP!

There are entire platforms and channels dedicated to sharing patient stories of how their lives were destroyed my SSRIs. And to add insult to injury, the psychiatrists don't stay silent... they call the patients liars and blame them... There are few things as corrupt as psych.
