@KevinGMayfield @ianmcnicoll For those who need assistance with monitoring they may need to be visited by District Nurses but this is just a ward round. PCN use of virtual wards is possibly different than the secondary care use. PCN use is keeping them out or escalating care quickly in an informed way.

@imperimus @ianmcnicoll I suppose what I really don’t like is the conversation focus. I’ve detected some link between ‘virtual wards’ and palantir. This is pushing a tech solution and may ignore what (tech support) is required to support patients at home.

What is the correct clinical term (not management term) for virtual ward? In FHIR, I don’t think this is ‘virtual’ but ‘home health’. A patient taking an observation at home is performing a home health event.

@KevinGMayfield @ianmcnicoll It would be a single encounter like an inpatient, observations will be taken automatically or by patient/helper for those given devices. Apps and 'how are you' questionnaire will also be used. The clinical team monitoring with occasional contact/clinical review (ward round).

@imperimus @ianmcnicoll I think I started leaning towards outpatient, as the patient is still being treated but is not in a physical bed and this follows the inpatient/emergency stay. The encounter methods maybe a mix of virtual ward rounds and traditional outpatient visits (inc virtual).

@ianmcnicoll @KevinGMayfield Would agree home inpatient or inpatient at home would be closer. However then you get into the debate of what classes as 'home'. Staying with friends, family or care home. It's the level of care and access to medical experts that is the differential; decentralised healthcare

@KevinGMayfield Outpatient/inpatient probs get driven by payments and not logical sense of the physical location. Hospital @ Home is the brand name here, which uses a virtual ward for managing visibility. Main difference to home health though is the direct escalation to clinical team.

I will argue all day long that ‘clinically led’ isn’t what creates successful digital transformation. For me it’s ensuring equity of voice and representation of all involved. Our ICS wide CxIO network includes non-clinicians and will include patient leaders.
