Lingering pathogens have been a big part of Chinese medicine, documented as early as 2,000 B.C. So much of the medicine operates around the interstices, ecapsulated spaces, and empirical evidence that lingering pathogens are not considered special or even rare.

In Chinese medicine, prolonged use of bitter-cold antivirals is thought to drive lingering pathogens deeper rather than eradicate them. Mechanistically, many can directly inhibit microbes while also suppressing the immune machinery needed to clear them. Shikonin and Berberine, for example, can inhibit dendritic-cell metabolism, maturation, and costimulation - reducing inflammation, but potentially promoting immune tolerance and impaired antigen presentation. This convergence between traditional “pathogen entrenchment” and modern APC biology is the subject of my book. Antimicrobials work great for acute pathogens when the host's immune mechanisms are still in tact, but Lingering Pathogens come with a world of complications that must be met with nuance and precision.

@internetuserf12 I was more vulnerable than the average perso, but saw the need for normality mentally so we went for it. As much as I didn't want Covid, balance was needed and I am so pleased I went back to normality. We had the vaccines - and a less severe virus. I've been fine.

Illness ghosting happens when someone’s illness outlasts other people’s willingness to care: friends and relatives abruptly disappear from their life. In oncology, it’s called "cancer ghosting" because it's so dramatic and painful; at its most extreme, it produces a kind of social death. That is precisely what the “return to normal” felt like to everyone still vulnerable to SARS‑CoV‑2: society’s concern and responsibility to each other expired before our illness and vulnerability did. In summary, it did not and does not feel good if you can imagine.

In the coming weeks, I’m replacing APC Restore capsules with a much more affordable organic freeze-dried raspberry electrolyte drink - more tolerable for more people and far less time-consuming to make. Creating a sustainable alternative to the no-holds-barred oxaloacetate capsule formula was always the goal. The capsule pilot brought encouraging results, but its cost, labor, and potential regulatory requirements make continued production unworkable at this scale. I'm excited to share the new formula!

@internetuserf12 I would also say even in mild ME there’s a degree of social death from the outset, being forced to choose between trying to sustain work vs socialising/ building and maintaining relationships.

I'm removing Oxaloacetate due to the price and Glutathione and Dihydroberberine due to their taste. I'm adding creatine, salt, magnesium glycinate, glycine, beetroot, and black carrot. It will be a touch more supportive for folks with dysautonomia without sacrificing a whole lot in terms of mitochondrial support.

What distinguishes LongCOVIDbot.com from ChatGPT and Claude is its closed, curated research library and 50 custom behavioral prompts. These act as both guardrails and incentives, organizing the evidence for stronger differential reasoning across diagnoses, treatment approaches, and cross-study synthesis while keeping claims transparent and cited to prevent hallucination(I'm continually improving it). The folks at Ultimo are working with me to scale for more features like persistent conversations and file uploads, launch dates pending. Again, I'm not a fan of AI so I do not take this project lightly and intend for it to be exemplary.

Chinese medicine is uniquely equipped to unpack complex chronic illness because it doesn't see the body as a collection of isolated organs or disease as a single broken pathway. It sees physiology as a network of perpetuating systems and disease as a network of perpetuating lesions. That framework is remarkably well suited to illnesses like Long Covid where multiple pathological processes operate both independently and interdependently to stabilize the disease state.

This paper adds another piece to my argument: impaired PBMC mitochondrial respiration during acute COVID-19 predicted greater Long Covid severity three years later.
