No, not unique—gymnastics, figure skating, and diving often need even earlier starts. But football's brutal: miss the academy boat by 10-12 and elite level is near impossible. Late bloomers like Drogba (pro at 21) or Ian Wright (21) exist, but they're rare exceptions. Start after 7? You can get good, even pro in lower leagues, but world-class? Extremely unlikely.

What if saving a pneumonia patient was as simple as adding one cheap pill?
A new NEJM RCT from Kenya just showed exactly that. 👇
2,180 adults with community-acquired pneumonia were randomized within 48 hrs to:
🅰️ Standard Care (WHO regimen)
β-lactam (penicillin/cephalosporin) + macrolide (erythro/azithro)
🅱️ Standard Care + 10 days of low-dose steroids
Dexamethasone 6 mg or
Hydrocortisone 160 mg or
Methylpred 30 mg or
Prednisolone 50 mg or
Prednisone 50 mg (bioequivalent dosages)
30-day mortality:
22.6% with steroids vs 26.0% with standard care
➡️ HR 0.84 (95% CI 0.73–0.97), P = 0.02
Adverse events similar. Steroid-related serious AEs: 0.5%.
💡 One low-dose steroid… in hospitals with almost no ICU support… saved lives.
Global pneumonia care may never be the same.

A) Dengue Hemorrhagic Fever
🎭Key Diagnosis
•Mosquito bite history → arboviral infection likely (Dengue/Chikungunya).
•High fever
•Petechiae / rash (“islands of white in a sea of red”) — characteristic of Dengue.
•Severe thrombocytopenia (Platelets = 40,000/mm³).
•Raised hematocrit → plasma leakage → hallmark of Dengue hemorrhagic fever (DHF).
🌀Differentiation in lines:
•🕸️Dengue Hemorrhagic Fever: High fever, petechial rash, thrombocytopenia, raised hematocrit due to plasma leakage, bleeding tendency.
•🕸️Chikungunya: High fever with severe joint pain (arthralgia), maculopapular rash, normal or mildly low platelets, no rise in hematocrit.
•🕸️Malaria: Fever with chills and rigors, splenomegaly, anemia, parasitic forms seen on peripheral smear, no rash.
•🕸️Typhoid: Step-ladder fever pattern, abdominal pain, rose spots on trunk, relative bradycardia, mild thrombocytopenia only.
🌀Key pointer:
Raised hematocrit + severe thrombocytopenia + petechiae → Dengue Hemorrhagic Fever.
