This type of marketing of protons doesn't instill confidence in our field IMO. The site is aptly titled askforprotons.com On it, there is a quiz w 4 questions (age, prior RT, tumor site), but no matter what answers you put, it tells you protons may be needed.

If TNT > preop chemoRT (PRODIGE, RAPIDO) and preop chemo = preop chemoRT (PROSPECT) it seems to logically follow that TNT > preop chemo. For most patients today, the standard of care is TNT which includes RT

Results took so long bc pts did so well on both arms. As the best studies do, this one leaves me with more questions than answers:

Ok, this exactly was my concern after the presentation of the PROSPECT trial at #ASCO23. It’s not „many patients with locally advanced rectal cancer“ it’s “Many patients who met the inclusion criteria of the PROSPECT study”. Longer thread to follow… @Medscape

Radiosurgery (25 Gy/1 fx) for tumor around celiac axis causing debilitating pain refractory to meds. 3 days later pain nearly gone, no side effects. "You gave me my life back!!" the patient told me this morning almost in tears. Today was a great day. @MiamiCancerInst @viewray

For our mets pts, we don’t worry about biliary tree and treat with 5 fxs. For HCC or cholangio pts we contour the CHBT and if we can’t stay under and get 50 Gy, drop to 15 fractions (with no biliary constraints).

I strongly recommend avoiding 3 or 1 # SBRT in central biliary ‘no fly zone’ ( not contoured) - 1.5cm around biliary tree
