Update - L radicular leg pain & foot drop almost resolved Unfortunately symptoms evolved to become bilateral gluteal, pelvic & groin - 2 weeks ago When the pain got to 9-10/10 & I couldn't sleep / walk, I decided to forego watching the Hungarian Grand Prix & head to Urgent Care at PGH instead - raised CRP & neutrophilia MRI on Sunday at Fortius (amazing service) - raging bacterial disciitis & epidural abscess Don't tell anyone but it's affecting the psoas so I can't SLR 😆 So now in hospital awaiting PICC line and 2-3 months antibiotics 😕❎

Update for those interested! 16 days post discectomy Minimal pain - low grade 2/10 neural ache buttock and fibula head / shin Worse at night and morning but variable Numbness is more annoying! Tib ant power recovering Doing simple upper body weights typically seated or cable Iso rehab for quad, hamstrings and tib ant Stationary bike & elliptical trainer - easy steady state I’m behaving and listening to my brilliant physio Chris Whittle @puresportsmed

‼️🦿Kept This one Quiet the last few weeks. Racing At the Olympic Games I TORE MY ACHILLES in the heats, didn’t want to make a fuss about it as I wanted to celebrate with my team mates winning our bronze medal. I suffered for months with my Achilles this year before finally tearing it in Paris. Returning home I got an MRI and I had a severely damaged & Torn Achilles tendon. This is my 3rd Achilles surgery but on my right leg which has always been ok. The last few years has been very rough but all worth it & will always put my body through the fire if it’s needed for success. Thank you do everyone who supported me through the Olympic Games you all now who you are & special thanks to one of the best surgeons in the world James Calder OBE for fixing me up yet again 🙏

@jpmrogers but a simple LA injection minimal risk and easy logistical option in clinic to rule in / out, Vs spinal injection ? (different level of intervention escalation ) Epidural could be organised quickly in the private sector if PHT eliminated .

Consent ✅ Middla aged Ironman Months of L inferior gluteal pain that radiates into posterior thigh to knee - burning / throbbing Recent 'pop' in gluteal region with sudden hip & lumbar flexion Worse on - running / fast walking, sitting, cough / sneeze +ive proximal hamstring tendinopathy tests +ive adductor magnus bias tests +ive SLR neural tension Pain LSp flexion, limited POCUS & MRI - interface tear (peel away lesion) of proximal hamstring but remaining tendons intact, so no retraction LSp MRI - L sided disc bulge irritating L S1 nerve root So - concurrent pain drivers? Or one injury mimicking the other? PHT pain can refer pain distally into hamstring. What next after 3 months rehab / load mods & no change? 🧐 Patient will return with symptoms provoked - then US guided LA only injection to proximal hamstring tendon - then re-test - easier & safer to rule out / in tendon driven symptoms first before considering spinal options.

I thought the records thing was a good bit of fun but if their research says otherwise then that’s fine. Parkrun’s about getting people moving. I hope this encourages more runners to get to get stuck into their local cross country league, road relays, club runs etc instead.

@DrJN_SportsMed Interesting case James. ?Differential of MALS - median actuate ligament syndrome causing mesenteric ischaemia but this usually has pattern of crescendo building over 10-15 minutes or more of sustained high intensity exercise

Consent ✅ Runner, burning abdominal pain when exercising, felt along lateral rectus (linea semilunaris) at level of umbilicus Possible ACNE syndrome (abdominal cutaneous nerve entrapment) Challenging to visualise nerve on US (see image) Diagram - correlating schematic of nerve passage through RA and entrapment point Video - US guided block (hard to visualise needle at depth but can see injectate filling area surrounding nerve)

@AbbasianAli @DrJN_SportsMed Understand where you are coming from but MRI does often help in cohort I see - recent 2nd metatarsal stress reaction in elite runner - offloaded for 10 days only (would have been 6 weeks + if fracture). ran PB and qualifying time which wouldn’t have happened without imaging
