Integrative Clinical and Molecular Evaluation of Renal Cell Carcinoma with Merlin Protein Deficiency and Biallelic Loss of NF2 aacrjournals.org/clincancerres/… NF2 loss in renal cell carcinoma defines an aggressive subtype with frequent metastases and poor outcomes. In a cohort of 33 patients, non-immunotherapy regimens showed numerically longer survival than immunotherapy, though not statistically significant. Genomic analyses revealed frequent NF2 mutations and alterations in cell cycle and DNA repair pathways. This is not a complicated #GUPath case. These prostate glands are "too many, too small, and too crowded" to be benign. They also "cross the street" (i.e. traverse the entire width of the bx core). Other soft signs of PCA which, imo, are very helpful are seen here: crystals & amorphous pink secretions. Nucleoli aren't as prominent as one would want them, but the amphophilic quality of the cytoplasm is another, often neglected clue for PCA. Dx. GS336 prostate carcinoma (#IHCPath is CK5/p63 showing lack of basal cells) Some cases are more subtle! These are the ones where the mantra "always trust the crystals" really comes in handy 😌 x.com/MirunaPopescu1…

Urothelium after intravesical mitomycin can show striking cytologic atypia—enlarged hyperchromatic nuclei, smudgy chromatin, and degenerative changes that may mimic carcinoma. Clinical history is key to avoid overcalling malignancy. Always correlate with recent treatment. #GUpath
