Thrombin generation often measured in plasma ▶️ low specificity for thrombin & limited utility in whole blood. Dai et al designed fluorescence resonance energy quenching–based thrombin sensor (FTS) protein, new point-of-care test⁉️ @platelet144 @kimpy79 jthjournal.org/article/S1538-…

@karimbrohi Kim PY, Stewart RJ, Lipson SM, Nesheim ME. The relative kinetics of clotting and lysis provide a biochemical rationale for the correlation between elevated fibrinogen and cardiovascular disease. J Thromb Haemost. 2007 Jun;5(6):1250-6. doi: 10.1111/j.1538-7836.2007.02426.x.

@karimbrohi We showed some time ago that when Fg is high, balance between clotting and lysis shifts to the former based on their individual kinetics ie higher Fg favours faster clot formation while rate of lysis does not change. May be helpful in rationalizing your findings?

Which makes no sense as the tendency in trauma is to give lots of stuff early to everyone. Unless... HIGH CONCENTRATIONS OF FIBRINOGEN ARE HARMFUL? If true, giving patients Fg who don't need it - because they haven't reached critically low levels yet - might be a very bad idea
