Everyone needs to calm down about women. We are past the "women are inferior and subordinate" stage. We are past the "all women should be a SAHM" stage. We are past the "women shouldn't vote" stage. Women aren't inferior to anyone. We are strong and capable in our own right. Women aren't trying to be men because men should not be the ideal we should strive for. We were meant for greater things. We have choices now. If a woman wants to be a SAHM mom, we support that. If she doesn't, we support that too. If she wants 10+ children, as long as it's her choice, we support it. If she wants to remain childless, she has our support. Women occupy space in this world, and we are going to continue to occupy spaces where decisions are made that involve us or affect us. Women have the right to vote and we are going to continue to vote. If you don't like the way we vote, keep in mind we aren't thrilled with the way you vote either. Maybe you are the problem. The more you attack feminism, the more we double down. We do not have rights because men allow us to. We had rights all along, men just kept them from us. Stay strong, ladies.

Ukazuje, že tyto infekce jsou v době úmrtí mnohem častější, než naznačují běžná klinická data. V průběhu čtyř chřipkových sezón byl u 36,4 % zemřelých prokázán respirační virus, přičemž chřipka byla přítomna v 11,0 %. Nejčastěji se jednalo o rhinovirus (11,4 %), chřipku (11,0 %), lidské koronaviry (7,1 %) a RSV (6,9 %), což ukazuje, že virová zátěž spojená s úmrtím zdaleka není omezena pouze na chřipku. Zásadním a klinicky velmi významným zjištěním je výrazná poddiagnostikovanost těchto infekcí během života. Podobný nesoulad se ukazuje i ve statistikách příčin úmrtí. Přestože byla chřipka post mortem zjištěna u 11 % zemřelých, jako příčina smrti byla uvedena pouze u 1,4 %. To podtrhuje, že respirační viry jsou jen zřídka uváděny jako základní příčina smrti, i když k úmrtí pravděpodobně významně přispívají. Často totiž nepůsobí jako primární diagnóza, ale jako spouštěč dekompenzace: kardiální, respirační nebo celkové. Zátěž byla zvláště výrazná u obyvatel zařízení dlouhodobé péče, kde byl respirační virus detekován u více než poloviny zemřelých (52,3 %), což odráží jak vysoké riziko přenosu, tak zvýšenou zranitelnost této populace. Byly patrné i sezónní výkyvy, s nejvyšší prevalencí v lednu a během vrcholu chřipkové sezóny, což odpovídá obdobím zvýšené úmrtnosti. Respirační viry byly přítomny napříč všemi kategoriemi příčin smrti, nejen u respiračních onemocnění. Zatímco chřipka byla častěji spojena s respiračními příčinami úmrtí, ostatní viry se vyskytovaly napříč různými kategoriemi, což podporuje představu, že virové infekce přispívají k mortalitě nepřímo, prostřednictvím různých orgánových systémů. Studie rovněž upozorňuje na koinfekce (přibližně u 11 % případů s prokázaným virem) a současnou cirkulaci více patogenů, což odráží komplexní virovou situaci během zimních měsíců. Je však třeba zmínit i určitá omezení. Samotný průkaz virové RNA neprokazuje příčinnou souvislost a může odrážet i asymptomatické vylučování nebo přetrvávající detekci viru. Studie navíc zahrnuje pouze chřipkové sezóny a přibližně 15 % všech úmrtí, což může vést k určité selekční bias. 💬 Komentář Jde o mimořádně důležitou studii. Respirační viry nejsou nevinnými „přihlížejícími“, ale významnými a často přehlíženými faktory přispívajícími k úmrtí. To, co běžně označujeme jako „kardiální příčinu“, „respirační selhání“ nebo „celkové zhoršení stavu“, má velmi často skrytý virový spouštěč. Chřipka, RSV i další respirační viry jsou výrazně poddiagnostikovány za života a zároveň nedostatečně uváděny v úmrtních listech. V důsledku toho systematicky podceňujeme jejich skutečný podíl na mortalitě. Pro klinickou praxi z toho plyne jasné sdělení: více testovat, častěji zvažovat virovou etiologii a brát respirační viry vážně, zejména u starších a křehkých pacientů. To, co se jeví jako dekompenzace chronického onemocnění, může být ve skutečnosti kolaps vyvolaný infekcí. A co je hlavně důležité? Očkovat, zejména rizikovou populaci.
