Here's a different kind of error. Generally, you're supposed to use only the *tip* of the Bovie to cut or coagulate (L photo). But sometimes people 'bury' the Bovie tip in the tissue, which could cause unintended damage. This is called 'past pointing' (R photo).

@rbarbosa91 Love your posts @rbarbosa91. Surprised to hear that the insulated is not used often. It prevents shorting on proximal skin/tissue. We @VanderbiltENT head and neck use the insulated tip bovies exclusively. Why wouldn't you want an insulated tip? @TopfHNS

There is an insulated Bovie tip which has plastic around the more proximal part of the Bovie tip and will limit the damage that can occur from 'past pointing'. The insulated tip actually isn't used that often, because surgeons know not to 'past point', but it does exist.
