@doclip1 I agree that T2-low is where we lack options and assessment of Teze on strictly T2-low would be very enlightning albeit difficult to recruit to as you highlight. IMHO a key step is moving away from a definition based on the absence of T2 to the presence of something.

Here we have an analysis of T2 biomarkers across a range of asthma severities from BREATHE cohort lnkd.in/eB2zjWSG   The main reason I am posting this is that I don’t resonate with the rather high prevalence (34%) of so called T2 low severe asthma…lnkd.in/ewvQJDuT

@doclip1 We have been cautious with the terminology in both papers ie "no biomarkers elevated" rather than "T2-low" for this exact reason.

@doclip1 I believe we agree on the prevalence of true T2-low - it is rare - and I do not believe the references Venn-diagram is fit to identify true T2-low as would lower cutoffs for both IgE and B-eos would be needed IMO.

Happy to see our research highlighted by the ERS! A key takeaway for daily practice is the need for routine airway inflammometry - not only in specialized SA clinics - as eosinophilia otherwise is overlooked in a large proportion of patients @RespBispebjerg @PluZdk @CPorsbjerg

T2 inflammation is highly prevalent, with biomarker co-expression a particular trait of severe asthma. The best treatable trait, eosinophilia, is missed in half of patients with mild–moderate and one-third with severe asthma without airway inflammometry. bit.ly/3rVPNbv

We find T2 inflammation to be more prevalent in SA and that biomarker expression relating to more than one pathway to be a particular trait of SA. Our findings also highlight the need for clinically feasible airway inflammometry as 22% of patients had airway eos without blood eos
