I would love to learn more about the white doctor in question, I really wonder about his practice that he feels so threatened by the idea that a patient may want to find a medical provider they feel may understand them and their needs better.

Examining Kate’s 1% She has suspected endometriosis. This affects at least 1 in 10 women, likely more. Here she’s getting an ultrasound. Historically you needed surgery just to diagnose it (incisions are made in the abdomen). We're doing a non-invasive route. Typically women live with endometriosis for 7-10 years before being diagnosed. It’s the leading reason women aged 30 to 34 get hysterectomies (permanent surgery to entirely remove the uterus). This condition is where endometrial-like tissue starts growing outside the uterus, in ovaries, bowel, bladder, even the diaphragm. This tissue inflames, scars, and glues organs together. Our first step is to find out if @_katetolo has it. Initial measurements we’re doing: + trans vaginal ultrasound + pelvic MRI w and w/o contrast + hormonal labs All during the early part of her cycle to get the clearest picture. During her ultrasound, a slim probe, about the width of two fingers, 10-12 inches long (although only a small portion is inserted) is covered with a protective sheath and lubricant and gently inserted into the vagina (patient has to empty their bladder first). This creates real-time images of the uterus, ovaries, and surrounding pelvic structures. While inserted, the probe is turned 90 degrees to evaluate all the various structures, angles and views. There is no radiation exposure. The technician is looking for scarring, ovarian cysts, adhesions, and for organs that are fused together with tissue. This ultrasound can confirm endometriosis but it cannot rule it out. What endo does to the body: + 90% report pelvic pain + 50% report severe fatigue + 26% report infertility. However many sources cite 30 to 50 percent. + 50% experience pain during sex. + Many have pain with ovulation, bowel movements, and urination + Severe bloating called “endo belly” where the abdomen visibly distends There are a handful of theories about why endometriosis develops but the honest answer is no one is quite sure. We’ll keep you posted on her results.

@DavidLBrownMD I can understand the desire for developing physicians with a certain level of grit but I don't understand a need for misery Olympics. Medicine really can be the least empathetic towards it's own. You have a kid now entering the field, would you want her to be treated the same?

When I was an intern I developed massive acute bronchospasm from exposure to cat dander. I ended up intubated in the ICU at one of my program’s teaching hospitals. Thanks to high dose steroids, I was extubated the next day. The nurse then handed me the phone to answer a call.

FACT-CHECK: As a psychiatrist, I regularly meet people living with schizophrenia and families trying to understand what their loved one is going through. Over the last few days, many have asked about a viral video claiming schizophrenia is not related to dopamine but caused by a “leaky thalamus” that overwhelms the brain with sensory information. The thalamus is definitely important. It helps filter and relay information in the brain; and disturbances in these pathways have been studied in schizophrenia for many years. Difficulties in sensory processing and attention may partly arise from these circuit abnormalities. At the same time, schizophrenia cannot be explained by a single mechanism. Dopamine remains important. Genetics, brain development, stress, trauma, inflammation and multiple brain networks are also involved. The condition is biologically complex. The video also gives the impression that medicines only “cover up” symptoms. That is not accurate. Antipsychotic medicines are not perfect and side effects can occur, but they have helped tons of patients regain stability, reduce hallucinations and paranoia, avoid relapse, continue education or work, and reconnect with family life. Research on metabolism, nutrition, exercise, sleep, and brain energy systems is growing and may improve future treatment approaches. These are promising areas of study, but they are not replacements for established treatment at present. Short social media videos often present one part of a much larger scientific picture as a complete explanation. Real clinical science is rarely that simple. It's like we never saw what the consequences of having mostly white male physicians did for every single other demographic in the US. DEI isn't against competency, it ensures a diversity of thought and ideas that lead to innovation that helps everyone.

I have a patient with glioblastoma who got halfway through a clinical trial and was seeing great results, only to have his treatment ceased when the trial stopped. He then spent $10,000 of his own money trying to use Expanded Access, and was denied. Now, accessing treatment through Montana's SB535 is his last hope. There are biotechs and manufacturers willing to provide treatment, but only if we can get guarantees they won't be punished by @US_FDA for doing so. I'll be in Washington D.C. with him on the 19th and 20th of this month, if anyone reading this would like to help, please tag your Senator and House Rep and ask them to make some time to meet with us.
