Very good read on the difficult differential between PNES& epilepsy. Great practical advise and discussion on the problems with suggestion and deception. In my experience, early and correct diagnosis of PNES is crucial, so make the initial eeg workup as comprehensive as possible.

DBS reduces seizures while not impairing cognitive performance in LGS. Impressive work by @drlindadalic et al. in a difficult study cohort! The paper also makes the important point that many clinical scores are useless in the assessment of patients with cognitive disability.

Cool paper on ultra long term EEG in epilepsy. If the seizure focus is nearby the electrode, sensitity for seizure detection seems to be pretty good (probably way better than seizure diaries). Whats the clinical impact? Diagnostic? Seizure monitoring?

Interesting paper out today. Fabian Wolpert and @GalovicMarian show high seizure frequency in patients with brain metastasis. Gross total resection was the only factor to reduce seizure rate. Is epilepsy surgery underused in brain tumor patients? doi.org/10.1111/epi.17…

@Brain1878 @pierre_vanmedge Sound and convincing ideas. Also in adults this is increasingly discussed, e.g. early surgery in cortical vascular malformation prior to drug resistance. But what are the optimal 'carefully selected' cases? And how to test this approach? Early versus late intervention?
