Once you become comfortable with morcellation of tumor, you can carry out #ENBLOC laser resection #ERBC also in case of big lesion like this (6cm). Almost bloodless surgery, wide exposure of detrusor muscle without perforation, superb coagulation. #THULIO #pulsedThuliumyag laser by @DornierMedTech definitely eases this surgery providing efficient & precise cutting combined with a superior coagulation without excessive carbonization! @piet_ost I typically have a discussion with these men. Sometimes get ctDNA. Systemic vs focal treatment. In this location, typically offer 5x SBRT. On a side note, this location is great for MR linac SBRT.

69-yo fit man, 3 years after RALP and adjuvant RT (pT3b pN1, ISUP 5) now with PSA recurrence with PSMA PET showing lesion in corpus cavernosus last year and now confirmed as growing. MRI confirmed. Q1: biopsy or not Q2: treat with RT (fractionation?) or systemic only?

Follow up🧵 He lives in Angola, Africa and visited many urologists that unanimously told him about a high risk of transfusion in such a large gland. All doctors demanded informed consent for transfusion, which is very difficult for them to sign due to their religious beliefs /1
