The power of randomization in open blinded trials….it gets almost 30% of patients with AS who were asymptomatic for years to be symptomatic in less than 6 months after randomization !!!! This is EARLY TAVR. nejm.org/doi/full/10.10…

Highlights of The Evolving Concept of Secondary Mitral Regurgitation Phenotypes: Lessons From the M-TEER Trials, just published in JACC: Cardiovascular Imaging: 1. Secondary mitral regurgitation (SMR) is a heterogeneous disease entity presenting with different clinical phenotypes 2. Differentiation of SMR phenotypes (aSMR vs vSMR) and their subentities might facilitate our understanding of treatment response to medial and interventional SMR treatment 3. My post-coronary angiography patient talk revisited after ORBITA and ORBITA-2: "Dear patient with chest pain and no pills, if I fix this artery you won't live one day longer, but you may have less angina, or you can take one or more pills and achieve more or less the same".

ORBITA-2, the revenge. In a randomized, placebo-controlled trial of 301 patients with stable angina who stopped all antianginal medications and had evidence of ischemia, the mean angina symptoms score at 12 weeks was lower with PCI than placebo. nejm.org/doi/full/10.10…
