So the baseline will gradually be suppressed to fund premia. And you've just made collective bargaining markedly more difficult...

You can of course do this - but don't think that it won't have unintended consequences. Specifically, you are right - you can have baseline + suppl. But while junior doc funding is complex,

@NadiaKamil The trouble is you’ve just created a good way to realise that their SSRI won’t do them much harm but that grannies x med otoh, will. To look at it another way, what proportion of the people you refer to were both a) entirely serious at the time b) impulsive or distressed?

I knew (from here) about being able to give 600mg Ibuprofen, but I didn't realise it could be QDS. It's this just my ignorance, or is there a culture of systemically under-dosing? Shouldn't we be going up to max NSAID dose before adding opiates?

@elagnab @trishgreenhalgh @wesstreeting I mean, yes, it is, but as we’ve established that equates to a 36-40 hr week. So what you’re asking for if you want them all to be available 5 days a week is for them to work 60-80 hrs. Doesn’t sound that sustainable does it?

@markjrussell @trishgreenhalgh @wesstreeting Yes I get that but I am trying to establish if the fact that there are x number of GPs but a significant proportion are only available to patients 3days a week is a factor in the pressure on appointments.

@elagnab @trishgreenhalgh @wesstreeting So the fact surgery website tells you that they are only available for consultations Mon-Wed doesn’t really help you know if they are p/t. If they arrive at 8, leave at 7, do an hour after kids in bed & spending Thurs am catching up on referrals/paperwork -> 40 hrs
