Around 12 noon today, a couple rushed into Medicine Casualty holding their 3-year-old. The child had fallen off a table; his wrist was visibly deformed, likely a clean break. He was crying inconsolably, surrounded by 4 or 5 frantic relatives. "Doctor, treat my child fast!" they shouted. I immediately assessed the situation. As a Medicine resident, I am neither an orthopedician nor a pediatrician. I knew that I was not in a position to manage this situation. I calmly told them: "This is Medicine Casualty. Please turn left, the Orthopedic Casualty is right there. Since you have many people with you, have one person take the child there immediately while another goes to the counter for a ticket so the doctors can order X-rays and meds." That’s when the shouting started. "You good-for-nothing doctors! Can’t you see this is an emergency? Why do we need a ticket?" My junior rushed over to help, only to be met with threats: "Treat him immediately or things will get bad for you." Then, the phones came out. Cameras were shoved in our faces. "Look at these doctors refusing to treat a child!" Here is the context a 30-second viral video would miss: 1. I was directing them to the correct specialist only 3 minutes away. 2. Without a hospital ID/ticket, the Ortho team can't even log an X-ray request into the system. 3. Staying in Medicine Casualty was actually delaying the child's care. To de-escalate, I eventually had to leave my own post unattended to personally escort them to the Ortho wing and explain the situation to the doctors there. Hours later, after the child was stabilized, the family returned to apologize. I appreciate the apology, but the damage of those "recorded" moments lingers. This is the daily reality for doctors: being vilified for following the very protocols that ensure a patient gets the right treatment from the right specialist. Before you share a "doctor refusing treatment" video, ask yourself: Are they refusing, or are they trying to get the patient to the help they actually need?

At IGMC Shimla, a doctor on duty is abused, threatened, and then physically attacked by a patient. The assault is recorded but only halfway. In the cropped clip truth is edited, context is erased and provocation is conveniently missing. The patient is seen kicking the doctor; the doctor in self defence retaliates. The outcome? The attacker walks free, while the doctor is suspended , An FIR is lodged not against the aggressor, but against the softest target in the room the man in a white coat and then comes the digital media trial: words like “thugs” and “looters” hurled at healers, as if hospitals are crime scenes and doctors are enemies of the state. In today’s India, evidence doesn’t need to be complete it just needs to be viral Truth is optional outrage is mandatory.

Few common mistakes that I have seen medicos doing while investing: 1. Starting late is probably the most common mistake. 2. Under exposure to equities in early phase of investing ( most funds go to FDs/ RDs due to fear of losing) 3. Buying stocks on tips 4. Having too many mutual funds of same category 5. Seeking F&O with busy schedule thinking that they will switch to investing once they make good corpus from trading 6. Buying ULIPs instead of term insurance plans. What else can you think of? I hope this helps people who are new into their investing journey.

India now mass produces doctors but refuses to create jobs for them A recent article published in Tamil Nadu had statements of junior doctors working as “Gig workers such as delivery agents” to survive and meet the family demands of their family who took loan for their medical education ! After 10 to 12 years of training ,young doctors are handed ₹15,000 to ₹30,000 salaries , wages that insult both intellect and sacrifice Meanwhile, some corporatised hospitals talk more about revenue targets than patient care turning healers into unwilling sales executives. This isn’t one state’s crisis but UP, Karnataka, Maharashtra, Bengal, Rajasthan . the story repeats everywhere with overworked, underpaid doctors pushed towards secondary job options A nation that cannot provide dignity to its doctors cannot guarantee safety to its citizens. When doctors break, the healthcare system collapses next the 6 decade old leaders fail to understand this ! When the stethoscope ends up hanging next to a delivery bag in Tamil Nadu it’s not a story it’s a symptom of a nation in medical freefall but @NMC_BHARAT and @MoHFW_INDIA wake up because for them conducting a fair exam itself is a challenge forget about a fair life to a doctor !

@aditya_gan3500 True sir, LMN usually causes persistent deficits. But in some neuromuscular disorders like CMT, fatigue induced conduction block and small muscle weakness can make handwriting initially normal but worsen with use. Maybe That’s why some patients report activity related decline.

Young lady presents to me with an unusual problem Her handwriting worsens after 2-3 pages of continuous writing. It is beautiful when she starts writing. Then her hand stiffens and tightens up and her handwriting changes so much that her teacher has accused her of getting other people to write her homework! No other problem. Motor strength in the upper limbs and sensory function is normal. 5th case of 1° writing dystonia I picked up. No stigmata of neuro-Wilson. I will obtain imaging and genetic studies to rule out a 1° torsion dystonia/other heredodegenerative condition. @aditya_gan3500 Sir cmt 1a is definitely most common world wide, but it is apparent that in south asia specifically in India cmt 4c is more prevalent. Unfortunately we do not have documented data yet but I am in group of cmt patients in India.more 4cers but with vast variety of symptoms too.
