You should always have an Advanced Directive to handle "end of life" decisions for when you are INCAPACITATED. Every stage has specific forms to use for those issues. It's wise to also have a Medical Power of Attorney form to designate a specific person to made medical decisions on your behalf when you are incapacitated (but should align with the patient's Advanced Directives). Additionally, you should have a form that specifically states "I am NOT an organ donor" found at HaloVoice.org
