On one hand, I’m very glad that all of these recent azelastine studies finally have more people talking about nasal sprays in a more positive light. They work, and they are an essential layer of SARS2 prophylaxis. But on the other hand, it’s important to keep in mind that a couple of successful trials does not necessarily mean that azelastine sprays offer superior protection to some of the sprays that we’ve been promoting since early in the pandemic (eg. Iota-carrageenan, nitric oxide, etc.) or that azelastine as an ingredient is specifically required. Azelastine just happened to be the ingredient able to get the funding, because it’s (sometimes, although generally shifting to OTC) still a prescription medication and has more backing from pharma interests. I think that azelastine sprays probably do work well as a prophylactic, and they’re a good option. But I would also caution folks because of the risk of drowsiness, because you are dose-limited by the drug itself, and because we also don’t fully understand the mechanism of action yet. As it stands, I’d only outright recommend it for people who have concurrent allergic rhinitis. A randomized clinical trial suggests that the antihistamine nasal spray azelastine may reduce the risk of COVID-19 and other respiratory infections when used regularly. ➡️ Participants using the spray were about three times less likely to get COVID-19 compared to placebo and also experienced shorter illness duration. ➡️ The effect is thought to relate to blocking viral entry in the nasal lining and reducing local inflammation, though the exact mechanism is not fully established. 1/
