We need coroners to involve the management and politicians that set up a system to fail patients. Rather than just putting frontline staff on the stand.

Powerful meeting this morning with @AmbulanceNAS who are working hard to ensure we have an Age Friendly Ambulance Service. Our members shared many insightful experiences. We are all looking forward to further developing this work

Goodhart's Law = when a measure becomes a target, it ceases to be a good measure Important concept and often a mistake made in healthcare Combat it by: 1. multi-dimensional measures 2. conduct thought exercises w/ various data to eval for unintended actions 3. regular review






Our National Human Factors in Patient Safety Conference is currently underway. We have two very interesting speakers, Professor Steven Yule and Dr Chris Turner delivering their talks to our attendees in the Albert Theatre in RCSI as well as online 👏
