Do a “near total” and leave as little gb behind as possible. Of course try to get the entire gb out, don’t just see a bad one a jump to near total. Also don’t injure the bile duct.

@WheatleyHousing Not great optics for your company when your driver panic lane changes at lights enough that my car emergency brakes, blocks the lane for a bit to “make a point” then starts yelling abuse out the window at my kids at the next junction. Need to do better!

Transverse choledochotomy interrupts fewer ductal arterioles and causes less ischemia, but it’s harder to extend for larger stones. Longitudinal incision allows better access but may compromise vascularity. Which incision do you prefer—longitudinal or transverse?

@jennifermov @Stentingwoman @EricKnauerMD 100%. Some patients have abdomens that you’d want to avoid going near at all costs. EUS drainage would likely be preferable in these situations (and hopefully provide a more durable solution than perc drainage).

@jennifermov @Stentingwoman @EricKnauerMD I do wonder though if the risks of a prolonged GA endoscopic procedure (that may require repeated sittings or interventions) are much different from a lap chole in a high risk patient. In my normal cohort, it’s generally the GA risk that’s the limiting factor.
