1/5 Dos RCTs confirman que el resectoscopio 22F reduce la incontinencia precoz frente al 26F estándar (6.5% vs 29.5%). La miniaturización en enucleación ya tiene evidencia de nivel 1. #HoLEP #EAU26
2/5 HoLEP, ThuLEP, AEEP, Aquablation... los resultados a largo plazo convergen cuando la técnica es anatómica. "Enucleation is enucleation" no es un eslogan — es lo que dicen los datos. #BPH
3/5 Tranexámico intraoperatorio en pacientes anticoagulados: OR 0.17 para complicaciones hemorrágicas. 932 pacientes, 30 centros. Dato clínicamente muy aplicable. #Urología #EAU26
4/5 El 35,9 % de pacientes candidatos a enucleación prioriza la eyaculación entre sus 3 primeros outcomes. El 14,8 % de urólogos no lo mencionan en el consentimiento. Dato incómodo pero necesario.

First ThuFLEP procedure successfully performed in Chile with Urolase+ Premium We are proud to share that the first #ThuFLEP procedure in Chile using the Urolase+Premium laser system has been successfully completed at @FelixHfb , with the strong support of our trusted partner IMV MED CHILE. The surgery was performed by Dr. @JIgnacioAcuna and involved a prostate of 50 cc with a non-simple anatomical configuration, further complicated by cystolithiasis with a 17 mm bladder stone. Despite these factors, the entire operation was completed in just 45 minutes. The patient was discharged within 24 hours, underscoring the efficiency and favorable recovery profile of the procedure. @JIgnacioAcuna highlighted the performance of the #DissectPulse mode, noting its precision in anatomical dissection, controlled tissue interaction, and effective hemostasis. According to his feedback, the laser performed excellently and efficiently throughout the case. Seeing more patients post-aquablation with persistent gross hematuria and LUTS.
Here are some endoscopic images post-aquablation (6mo to a year ago). Most impressively, one patient had fused vessels across the aqua defect.

100 cc prostate with irritative symptoms 18 mos post urolift elsewhere. After enucleating the lateral lobes and depositing into the bladder it’s apparent the clips were projecting into the bladder. I am glad HoLEP can easily correct this condition. @EndourologyNm @NM_Urology
