I’ve been reading/watching about the Harold Shipman case, and it made me think a lot about safeguarding because it’s a really good example of how important safeguarding is. One of the first people to raise concerns about Shipman was Linda Reynolds and she offered two explanations for her concerns “The first was that shipman was a caring doctor, who looked after his elderly and sick patients, in their own homes, rather than having them admitted to hospital, and who visited them frequently when he knew them to be ill. The second was that he was killing his patients” (From the Shipman report) The flags she was seeing, along with a female coroner who was concerned as well, were the disproportionate number of deaths in his patients, and the fact they were largely women of around the same age instead of a demographic mix. Added to that, many victims were found fully dressed in their own homes, rather than sick in bed which is more usual in deaths after illness. They were not surrounded by friends and relatives either, like many dying patients. They were also often ‘found’ by him. Or he was present at their deaths. Which seemed odd. What’s interesting is that these flags are significant, and turned out to be correct, but not concrete at the time she raised them I think too many people still think safeguarding concerns are only legitimate when concrete facts have already been discovered. But safeguarding is a protective tool that actually opens the door to gathering facts because of things that flag up or feel off. Raising concerns is not defamatory or cruel, just because you can’t yet prove they are correct. Raising concerns is how you protect vulnerable people until the facts are known. And that only works if people take it seriously enough, and investigate, when flags are raised He was considered a “lovely” man by many, and he had a lot of patients who trusted and liked him. In acting on her suspicions, despite that, she not only did the right thing but took a certain amount of personal risk. If the police had verified her concern when she brought it to them, three more women would have been saved And in a parallel universe, where an innocent doctor was flagged up for similar reasons, and found to be fine, it would still have been the right thing for her to step forward No matter how lovely he seemed

@MentalHealthCop I was listing to the director of Nottingham social services this morning and I was absolutely astounded. Surely it’s an incumbent on them to work as a whole system to know who is in hospital and who needs a section 117 assessment. It’s their literal duty.

I have spent much of this week thinking about just how much dysfunction and damage is inflicted on our criminal justice / mental health systems by unilateral actions and omissions by leaders who fail to see the whole thing as just ONE system. Hard to see past it.

That litigation was ever EVER required to sustain this elementary point, this basic working assumption of any society that even superficially espouses the principle that women have a right to real, practically effective and substantive equality with men. Those responsible in government, public bodies, parliaments, political parties, trade unions, universities and others, including regulators, must now answer for the malign regression of women's rights over which they have presided in the UK for the last decade and more. @wendyburn @AgnesAyton What is being revealed through this enquiry is that clinicians have very limited time to work with people with the most severe mental health conditions. They need a lot of care and support. Mental health services do not have the resources or indeed the training and expertise

@wendyburn @AgnesAyton Of course, of course it’s devastating for any health or social care professional to be involved in such a terrible incident such as this. However, the truth must be revealed and there were significant failings in relation to very basic clinical care.

Nach fast drei Jahrzehnten im künstlichen Exil meines eigenen Körpers ist dieser Weg zu Ende. Ich stehe nicht hier als Opfer von gestern, sondern als unerschrockener Zeuge für morgen. Ich habe den Abgrund nicht nur gesehen, ich habe in ihm gewohnt – ich habe seine Kälte geatmet, bis ich vergass, wie sich das Leben anfühlt. Jetzt bin ich zurückgekehrt, um der Leere ihren wahren Namen zu geben. Wir leben in einer Ära, die den Schmerz der Seele mit dem Skalpell exorzieren will. Man hat uns gelehrt, Identität sei ein Konstrukt, das man operieren und chemisch erzwingen kann. Doch ich habe gelernt: Man kann die Biologie zum Schweigen bringen, aber man kann sie nicht besiegen. Mein Körper war nie „falsch“ – er war der Schauplatz einer tiefen inneren Not, die Halt suchte und stattdessen Medizin fand. Wir haben vergessen, dass eine Wunde im Geist nicht durch eine Narbe auf der Haut heilt. Denn am Ende bleibt eine unerbittliche Gewissheit: Die Natur lässt sich nicht betrügen. "Der wahre Verrat an der Jugend ist nicht die fehlende Bestätigung. Es ist die Feigheit der Erwachsenen, den Kindern die Wahrheit zuzumuten." Dass Leben bedeutet Reifung und kein chirurgisches Projekt. Wir opfern die Integrität gesunder Körper auf dem Altar einer flüchtigen Ideologie. Wir stehlen ihnen die Pubertät, diesen heiligen, schmerzhaften Prozess des Werdens, und fesseln sie stattdessen an eine lebenslange Abhängigkeit von der Chemie. Haben wir als Gesellschaft wirklich den Verstand so weit verloren, dass wir den Verlust ihrer Fruchtbarkeit als Freiheit und ihre Verstümmelung als Selbstfindung feiern? Das ist kein Fortschritt, das ist die systematische Hinrichtung der menschlichen Natur. Ich blicke heute mit einer Klarheit in den Spiegel, die keine Hormone jemals erzeugen konnten. Es ist die unbeugsame Erkenntnis, dass wahre Freiheit nicht im Wechsel des Geschlechts liegt, sondern in der Kraft, es anzunehmen. Echte Erlösung findet man nicht im Entwurf eines neuen "Ichs", sondern in der Kapitulation vor der eigenen Wirklichkeit. Erst als ich aufhörte, den Krieg gegen meine eigene Natur zu führen, begann ich wirklich zu atmen. Ich habe gelernt, dass das Privileg deines Lebens darin besteht, zu werden, wer du wirklich bist – nicht, wer du vorgibst zu sein. Diese Jahrzehnte der Selbstentfremdung sind für mich kein blosser Verlust, sie sind ein Mahnmal. Ein Aufruf zur Umkehr, die Biologie wieder als Heimathafen zu begreifen. Wer heute schweigt, während Kinder in die lebenslange Medizinisierung geführt werden, hat das Wesen der Menschlichkeit verraten. Die Wahrheit braucht keine Mehrheiten – sie braucht nur jemanden, der sie ausspricht. Ich stehe heute hier, weil ich nicht aufhören werde, das Schweigen über diesen Weg zu brechen. @nuwandiss Yes, I think that would’ve been good to understand the threshold for forensic community services. I’m also puzzled as to why people keep referring to the RC when VC was not under any framework of the MHA in the community.

@annedil20288403 @NFMHSIreland No disrespect to forensic psychiatrists but they have 3 potential advantages: 1. The risk has already materialised into actual behaviour 2. They often have more info than General Adult psychiatrists 3. They may be treating patients with greater environmental/MHA control
