Most of the literature is surprisingly unhelpful here, focusing on the macro effects (essentially just that there IS a finite ability to dilute urine and if water intake exceeds what can be cleared, hyponatremia occurs), but doesn't attempt to dive into the tubular level...

Was asked a Q about the renal physiologic mechanism of low solute (eg tea-and-toast or alcoholic) hyponatremia, and after dithering realized I don't really know on the nephron level why inadequate solute limits ability to dilute urine. Help? @kidney_boy @Caulimovirus #NephTwitter

@critconcepts @kidney_boy @Caulimovirus @kidney_boy gives a great discussion on it through @thecurbsiders podcast I think! Kidneys need the salt to create enough UOP (with free water excretion) to keep up with total free water intake! Also lower gfr -> less ability to dilute urine

@katiewiskar Why is RVSP underestimated in significant TR? Due to assumptions in modified Bernoulli or because the jet peak velocity is less accurately measured?

Elevated RVSP, almost certainly underestimated in the setting of torrential TR:
