🚨 IT'S LIVE! 🎙️ SoCCC Pre-Rounds — Ep. 12: Vasoactive Medications in the CICU Picking a pressor in cardiogenic shock isn't just algorithm-following — it's pharmacology, evidence, and pattern recognition working together at the bedside. Dr. Anthony Carnicelli and @ElliottMillerMD break down the real-world decision-making behind vasopressors and inotropes in the CICU: norepinephrine, dopamine, epinephrine, vasopressin, phenylephrine, dobutamine, and milrinone. They dig into: 🔹 A stepwise framework for choosing the right agent 🔹 What SOAP II and DOREMI actually tell us (and don't) 🔹 Tricky scenarios: LVOT obstruction, severe AS, pulm HTN, arrhythmogenic shock 🔹 When escalating pressor needs mean it's time to call the shock team Whether you're pre-rounding before sunrise or managing a crashing patient overnight, this one's built for immediate bedside application. 🎧 Listen now: open.spotify.com/show/0TFSW3CgP… Made possible by unrestricted support from @zoll_cms #CardiogenicShock #CICU #MedEd #CardiologyFellows #CriticalCare #FOAMed #Cardiology #IMResidency #MedTwitter @CCCEnthusiasts @JasonKatzMD @jameshorowitzmd @AnnGageMD

🫀 Episode 11 of SoCCC Pre-Rounds is live. Is the evidence finally catching up to the technology? After more than two decades without a major positive randomized trial in cardiogenic shock, DanGer Shock changed the conversation. Dr. Simon Parlow @sdparlow sits down with Dr. Jacob Møller — critical care cardiologist, professor at University Hospital Copenhagen, and one of the key minds behind this landmark trial — to unpack what it means for clinicians right now. This isn't just a trial recap. It's a candid look at how persistence, collaboration, and a bit of luck drove a breakthrough in a field that had been waiting for one for far too long. "I'm working in a very small cardiac ICU; we only have seven beds, but we randomized more than 100 patients with out-of-hospital cardiac arrest every year." — Dr. Jacob Møller 🎧 Listen now: open.spotify.com/show/0TFSW3CgP… @CCCEnthusiasts @JasonKatzMD @jameshorowitzmd @AnnGageMD @ElliottMillerMD Supported by @zoll_cms — keeping high-impact education free. #CardiogenicShock #CriticalCare #Cardiology #DanGerShock #MedTwitter #CICU #CardiacICU #PodcastForClinicians

🫀 New Episode | SoCCC Pre-Rounds 11 | Dropping June 5 at 5 PM ET For over 20 years, cardiogenic shock lacked a major positive randomized trial. DanGer Shock changed that. Dr. Simon Parlow @sdparlow sits down with Dr. Jacob Møller — critical care cardiologist at University Hospital Copenhagen — to break down what microaxial flow pumps actually do at the bedside, why the DanGer Shock results surprised even the investigators, and how patient selection determines everything. Key insights from this episode: → How these devices unload the left ventricle and reduce myocardial oxygen demand → Why only a small fraction of shock patients qualified — and what that means for you → Lactate trajectories as a guide to device management → Why the weaning window may be earlier than we thought (12–24 hrs) → Applying DanGer lessons outside the trial population "After seeing the results of DanGer, we have probably become a little bit more liberal in using the device in other forms of ischemic shock." — Dr. Jacob Møller 🎧 Listen June 5 at 5 PM ET — join the SoCCC community: soccc.org/subscribe @CCCEnthusiasts @JasonKatzMD @jameshorowitzmd @AnnGageMD @ElliottMillerMD Supported by @zoll_cms — keeping high-impact education free. #CardiogenicShock #CriticalCare #Cardiology #DanGerShock #MedTwitter #CICU #CardiacICU #PodcastForClinicians

American Heart Association@American_Heart·
The American Heart Association mourns the passing of the legendary cardiologist Eugene Braunwald, M.D., widely recognized as one of the most influential figures in the history of cardiovascular medicine. Over seven decades, his work reshaped the understanding and treatment of heart disease, leading many to call him the father of modern cardiology. Braunwald was a lifelong contributor to the American Heart Association, helping advance its research and scientific mission, and was honored with some of the Association’s highest honors for his lasting influence on cardiovascular care and research. His influence extended well beyond his own discoveries, as generations of Association‑supported investigators, clinicians and academic leaders were trained by Braunwald or guided by the clinical trial standards and mentorship models he helped establish. newsroom.heart.org/news/american-…

🎧 Available Now: SoCCC Pre-Rounds – Episode 9 The POCUS Revolution: Why Echo Belongs in Every ICU With Dr. Hatem Soliman @EchoSoliman Hosted by Dr. Simon Parlow @sdparlow Point-of-care ultrasound is transforming resuscitation and cardiac critical care—and in this episode, we break down how to use it effectively at the bedside. Join Dr. Simon Parlow and Dr. Hatem Soliman as they explore how critical care echocardiography can identify reversible causes of cardiac arrest, guide CPR, and refine real-time hemodynamic assessment. 🔍 Key takeaways: • Look beyond the IVC—multi-organ Doppler for venous congestion • Interpret cardiac output in context, not isolation • Master high-yield views like the apical 5-chamber & short-axis great vessels • Recognize common pitfalls in POCUS If you’re aiming to sharpen your resuscitation and ICU skillset, this episode delivers practical, high-impact insights. 🎙️ Listen now: open.spotify.com/show/0TFSW3CgP… @CCCEnthusiasts @JasonKatzMD @ElliottMillerMD @jameshorowitzmd @AnnGageMD #POCUS #CriticalCare #Cardiology #Echo #MedEd #SoCCC #CardiacICU

In an international, randomized trial involving patients with acute venous thromboembolism, the risk of clinically relevant bleeding was significantly lower with apixaban than with rivaroxaban during the 3-month treatment period. Full COBRRA trial results: nejm.org/doi/full/10.10…

Median one-yr cost of $37,913 USD (3X higher than cost of avg HF and AMI admission in Canada). Cost was $63,539 for those readmitted to hospital and $77,643 in lowest home time quartile. Clearly reducing disability and maintaining independence is key for cost-effectiveness!
