depends where you are - North East it means "I've acknowledged you as a human being now say the same thing back and leave me alone" down south it's likely genuine and jovial, respond with a short emotional feeling, "doing good today, how bout you?" take a look at their face see if the expression is genuine or nonplussed a standard humurouse used sarcastically response is "living the dream"
