My first patient today has diabetes and is having trouble with their blood sugars. We started discussing what they ate for breakfast and they had matzah with peanut butter. Voluntary matzah The day before Pesach Dude, WTF?

Just had a patient get jardiance declined after his prior insurer insisted on it instead of farxiga. The new insurer United Healthcare said I should use metformin instead. He isn't a diabetic.

@GarySingerMD A number of my dialysis patients have inadequate access to grocery stores and healthy foods (food deserts) and can only eat what is available to them in shelters and through food banks. These are often foods high in K and pre dialysis K runs very high. 1K often needed.

@NephRodby Depends. The default is actual body weight, but if the patient is not meeting goal (more common for those with higher weights who are feeling well and metabolic parameters are on target, will do some version of IBW (somewhere close to the average of IBW/actual body weight).

PD docs when you measure weekly KT/V urea do you use patient's actual body weight or ideal body weight for estimating total body water (V)? (Doing this can raise the KT/V in larger patients and lower it in thinner patients).
