I just happens when you are close to veins. All liver surgeons encountered this during complicated procedures. However, I think that risk is lower when you dissect from the top along the veins, starting at the confluence.

In addition we had to take the coronary / left gastric vein. Thus, if we didn’t reimplant the splenic vein then the stomach and spleen would develop venous congestion.
