Sharing this video of a strangulated recurrent left inguinal hernia with ischemic small bowel requiring resection and anastomosis. Because of the contaminated field, I decided not to repair the hernia defect during the index operation. Different experts. Different answers. Same patient. In this case I shared — a strangulated small bowel in a woman in her 60s — opinions were divided between resecting vs preserving. The bowel was concerning. I decided to continue lysis of adhesions to allow more time for recovery, and then performed the final evaluation. With preserved peristalsis, good mesenteric pulsation, and adequate ICG/Firefly enhancement, I chose to preserve. I explained the risks to her and her husband. She was discharged on POD2 tolerating diet. This case was 6 months ago, and she remains completely asymptomatic. That’s why surgery is not algorithmic. It’s clinical judgment, timing, physiology, and experience. AI and robotics enhance us — they do not replace surgical decision-making.

I would like to share a video of the laparoscopic management of a small bowel volvulus caused by a dense adhesion. The use of ICG is a useful tool to assess bowel perfusion in a compromised segment, allowing real-time viability evaluation and avoiding unnecessary small bowel resection.
