31 yo male with Hx of Sydenham's chorea @ age 14 (resolved after 2 yrs). Questionable APS Dx at that time. No Hx of Rheumatic fever. Recent 1 hr episode of confusion+amnesia in another city.MRI Brain-No infarcts. Started on Aspirin 81mg. Exam unremarkable. Echo Normal. APS Labs

Interesting question! hasn’t come up in my practice. I found 1 similar case report. I think shared decision-making/risk benefit discussion. intrathoracic pressure increased with trombone playing...But if this is his job perhaps benefit outweighs risk pubmed.ncbi.nlm.nih.gov/10994010/

Treating a patient with ICA dissection, non traumatic. He is recovering well with standard care / residual Horner syndrome only. Pt is a 60 yo male trombone player. Any thoughts about restrictions wind instruments playing @heatherlgornik @EstherSHKimMD thanks
