For the past 15 years, I have argued that the diaphragm should be investigated, if only to rule it out as a contributing/causative factor in SIDS. For 15 years, I followed the formal process of publishing articles in peer reviewed journals, and submitting commentaries and letters to the editor (see PubMed). I never considered twitter/x an appropriate forum. Over time, publishing got progressively harder due to the entirely valid, “your previous article made the same point /lack of novelty” critique. This was one of the main reasons for my latest 11 rejections, but Editorials don’t need to be novel, just salient. Over the years, I shared the peer-reviewed publications w many experts and presented in a few scientific conferences that would have me (usually just before the coffee break). For 15 years, I was politely ignored and lost hope more times than I care to remember. But like a stubborn boomerang, the dilemma always came back. I have been spectacularly unsuccessful in raising awareness or propelling action: no new clinical or pathological data on SIDS & diaphragm has been published since my first article in 2011, or indeed since the late 1990’s. It’s beyond obvious that the question of the possible role of the diaphragm & SIDS won’t be resolved on twitter/x. It can only be addressed by experts who conduct clinical/pathological/etc research and publish the results in peer reviewed journals. However, I have come to accept that Twitter/x might play a role in raising awareness of this blind spot among experts (neonatal critical care, pediatric and respiratory medicine, SIDS, etc), and that is why, after much hesitation, I published my first tweet a few weeks ago. I can only hope that these X posts reach the appropriate experts so they can critically evaluate the existing evidence and hopefully, over time, publish data to either invalidate or validate the hypothesis.
