@JAMASurgery Minimizing percutaneous drainage time should be a top priority! Internal endoscopic drainage is consistently shown to be superior and is a large gap in surgical education/training. Perc drainage is not benign!

@SChandanMD Thanks Saurabh! I prefer overstitch for full thickness bites. This was a standard duodenoscope - TJF-Q190V. I’ve gotten into the habit of trying to place the LAMS in the proximal/mid body and dilating the pylorus as needed to ease scope passage.

One-stage EDGE performed for a RNY pt with symptomatic choledocholithiasis. Transgastric access obtained w/ LAMS → stent fixation → ERCP with duct clearance. A reliable option for RNY anatomy that avoids the long-limb ERCP challenges in altered anatomy. #GITwitter #ERCP #EUS

@muhammadbaigmd Thank you! Patient had a slightly torturous CBD requiring EUS to be in a relatively unstable position to get good access. Patient also had partial GOO, want to reserve ability for future stenting if EUS-GE cannot be done.
