Thoughts from the couch….. Imagine you are an infant who does not yet understand that you and your caregiver are separate people. According to Melanie Klein and Object Relations theory, this early stage of life is dominated by persecutory anxiety, which is a primitive and life-or-death terror. Because a baby’s brain cannot handle the idea that the same person who feeds them is also the person who occasionally makes them wait or feel hungry, they use a mental survival trick called splitting. They push all their frustration and fear outward, essentially convincing themselves that there is a bad force out there trying to destroy them. It is not a logical fear but a biological alarm bell ringing to keep the good parts of themselves safe from the bad parts they have projected onto the world. As we grow up, most of us learn to bridge this gap by realizing people are a mix of good and bad, a process known as integration. However, for individuals struggling with personality pathology, that bridge is fragile. When they feel stressed or vulnerable, they fall back into that infant-like state of persecutory anxiety. They might suddenly view a friend or therapist as a persecutor who is intentionally trying to humiliate or abandon them. In their mind, a neutral event like a late text message is not just a mistake but a targeted attack. They use defenses like projection, such as accusing you of being angry when they are actually the ones feeling it, because admitting their own bad feelings feels like they might literally fall apart. In a clinical setting, the goal is to help a person move from this paranoid state toward what Klein called the depressive position, which actually represents emotional maturity. This does not mean being sad but it means being able to feel depressive anxiety, which is the healthy worry that your own anger might hurt someone you love. Instead of seeing the world as a battlefield of heroes and villains, the person starts to tolerate the gray areas of life. Therapy provides a space where the clinician acts as a container, holding onto all those intense and projected fears without getting angry or pulling away. Over time, this helps the individual realize that they can survive mixed emotions and that bad moments do not have to mean the end of the world.

#WorldAutismDay 2nd April Autism is recognised under the Rights of Persons with Disabilities Act, 2016. Awareness is growing. Access to certification and support still lags behind. A diagnosis is only the first step. A disability certificate is what opens doors to support, services and rights. Many families miss this link. Here is the pathway-clear and practical: Step 1: Notice early signs and seek evaluation Watch for: • Poor or inconsistent eye contact • Limited social interaction • Delay in speech or no meaningful words • Not responding to name • Limited gestures like pointing or waving • Repetitive behaviours (hand flapping, lining objects) • Strong need for routines A psychiatrist/pediatrician confirms the diagnosis. Step 2: Apply for disability certification (UDID)- Apply via swavlambancard.gov.in through AADHAR based ID after diagnosis at a government hospital from psychiatrist/paediatrician. Keep ID proof, address proof, photographs and medical records ready. Step 3: Visit the designated government hospital- Attend the assigned district hospital, medical college or notified institute. Certification is issued only through the government system. Step 4: Disability board assessment- Evaluation is done by a specialist team. Disability is quantified using the Indian Scale for Assessment of Autism (ISAA). Presented before Disability Board of hospital for verification of findings of specialist team. Step 5: Disability percentage is determined ISAA score is converted into percentage. 40% or more = benchmark disability, required for Social Welfare benefits. Step 6: Certificate and UDID card issuance- The disability certificate is uploaded by the Disability board of hospital on portal swavlambancard.gov.in A UDID card can be generated by caregiver through Mobile OTP verification Step 7: Use the certificate It enables access to: • School support and accommodations • Disability schemes and financial benefits • Reservations as per law • Rehabilitation services The gap is not in policy. The gap is in awareness and timely action. A child should not lose years of support because the process was not known. Diagnosis helps you understand the child. I am Dr. Sivaranjani, a pediatrician , raising concerns about how ERZL is being marketed and positioned in pharmacies, and the risk it creates for consumers making critical decisions during dehydration. This issue is not about whether ERZL is safe in isolation.
It is about how it is presented, perceived, and potentially misunderstood in real-world settings. ERZL is a commercially marketed electrolyte drink. However, its branding and advertising raise serious concerns. *It should not be using ORSL in its communication.* Why? Because: * ORSL itself is not the original medically recommended ORS * Referencing ORSL in any form creates brand recall in the minds of consumers * Instead of reducing confusion, it continues and reinforces it When ERZL is positioned as a continuation or replacement of ORSL, it keeps that same association alive — especially among parents who may already believe ORSL was equivalent to ORS. 👉 This defeats the very purpose of regulatory action. In pharmacy settings: * People are often making quick decisions under stress * They rely on familiar names and visual cues * Branding can directly influence what they choose If ERZL continues to build on ORSL recall, it risks being perceived as a medical solution for dehydration, even when it is not the same as ORS (the medically recommended solution). There are also concerns around ingredients like sucralose, especially in children and with prolonged use, based on global health guidance. This makes clear communication even more important. After raising these concerns, I have received a legal notice from Johnson & Johnson and Kenvue. This petition is not about conflict. It is about: * Clarity in healthcare communication * Preventing consumer confusion * Ensuring responsible branding and advertising * Allowing doctors to raise public health concerns without hesitation We urge the Food Safety and Standards Authority of India and the Ministry of Health and Family Welfare to: 1 *. Ensure that, since ERZL closely resembles ORSL, Kenvue is directed to rebrand it in a way that does not create recall or association with ORSL.* 2. Prevent misleading positioning that may confuse consumers 3. Strengthen enforcement at the pharmacy level 4. Ensure clear differentiation from medically recommended ORS 5. Protect healthcare professionals raising genuine concerns In healthcare, even small confusion can have big consequences.
