Clinical practice guidelines take years to update. The literature doesn’t pause in between. We worked with @AAOHNS to build a methodology that continuously monitors emerging evidence against existing guideline recommendations and flags where the science has moved. They’re the first medical society to pilot it. The goal: the recommendation you rely on today reflects the evidence available today.

Our cofounder & CTO Zack Ziegler keynoted @PMWCintl yesterday. His talk started from one fact: ~half of US physicians use OpenEvidence. How do we do good for the world in ways that are only possible because of the scale of usage? His closing thought: "In 2024 and the first half of 2025 we saw the linear impact regime of AI in healthcare. At the scale we are at today, we have entered the super-linear regime, where increased adoption leads to better outcomes with impact that contributes more than the sum of its parts." The most interesting part of the story is about to start.

@kleinerperkins was the first venture capital firm I ever heard of, when I read the Google book as a kid growing up in India. It had a constant halo in the back of my mind, even though I didn't know much about technology or investing at the time. I naturally jumped at the chance to apply to the @kp_fellows program in college and was fortunate to be a part of the 2014 engineering class. Things came full circle when I joined the investment team in May 2022. Data centers dominate AI, but they're hitting physical limits. What if the future of AI isn't just bigger data centers, but local intelligence in our hands? The viability of local AI depends on intelligence efficiency. To measure this, we propose intelligence per watt (IPW): intelligence delivered (capabilities) per unit of power consumed (efficiency). Today’s Local LMs already handle 88.7% of single-turn chat and reasoning queries, with local IPW improving 5.3× in 2 years—driven by better models (3.2×) and better accelerators (1.7×). As local IPW improves, a meaningful fraction of workloads can shift from centralized infrastructure to local compute, with IPW serving as the critical metric for tracking this transition. (1/N)
